Non-urgent advice: Register as a New Patient

Please complete the form below . These are also available from reception if you do not want to fill these online. Please note that we only accept patients with postcodes starting with M44 and M30. If your postcode is outside of our boarder then please speak to a member of the senior team before completing your registration forms.

Once you have been accepted as a patient You Will be assigned a GP,  your medical records will be transferred to the practice and you will be sent a new medical card. Your medical records may take several weeks to arrive at the practice.

If you wish to change your assinged GP you can do that by calling 0161 775 7373

We advise all our new patients to make a routine health check with one of our nurses within the first month of joining our practice.

If English is not your first language, the practice has access to interpreters. Please inform reception staff and this assistance can be provided.

Temporary & Overseas Patient Registration
 
 

UK Residents:

If you are in the area for a short time and require urgent medical treatment you are able to register with this practice as a temporary resident. This gives you almost the same access to general medical services as registered patients but only for a period of up to three months. If you are going to be in the area for longer than three months you should register with a GP.
 
If you are registered with another practice locally it is essential that you to attend your own practice, where your medical notes are available.
 
 
As a temporary patients you will not be eligible for Travel Vaccinations.
This service can be provided by private travel clinics or by your registered GP Practice
 
 

Emergency Treatment for Foreign Visitors:

People from abroad are eligible for NHS treatment in the case of an emergency. Broadly, this means that if, in the opinion of the GP, your condition requires emergency treatment you will not be charged for a consultation with the GP.
 
If you require treatment or medication for a pre-existing condition you will be charged a fee, which then should be claimed back via your insurance company.
You can receive emergency treatment for up to 14 days.
 
Please note that these conditions also apply to British nationals living abroad.
 
Should you be registered with another practice locally you are required to attend your own practice, where your medical notes are available.
 
Disabled Patient Facilities

Disabled patient facilities are also available at this practice. The practice is accessible on one floor, we have an automatic front entrance door, accessible car park space and toilet. If you require assistance then please let one of the staff members know and we will be happy to assist you.
